I confirm that I have provided and included any skin conditions or medical history in my booking notes above and are accurate and complete.

Aftercare Compliance:

I agree to follow the aftercare instructions provided by the Gentlemen's Chair Barber Shop to ensure optimal results.

Media Release:

I consent to before-and-after photos being taken for salon records or social media. I understand I can change my mind at anytime by notifying Gentlemen's Chair Barber Shop in writing, adding this request in my booking notes and checking if this has been updated in my customer card.

Service Time Allocation:

I understand that the Service Time Allocation is an estimate only. The allocated service time may be shortened or extended depending on the client's individual needs and the service selected. Any adjustments to the service time will be determined at the Barber's discretion.
